• Introduction
The Town of Bethel, Park and Recreation Department is requesting proposals for brick and mason work for repairs and improvements to downtown sidewalks on Greenwood Ave.
• Project Summary
The Town of Bethel, Park and Recreation plans to carry out repairs and improvements to beautify the Greenwood Ave.
• Scope of Work
• Take up paver walkways on Greenwood Ave and reset.
Location 1 – Start at Depot Place to Rector Street.
Location 2 – Start at the cross walk and bench in front of the library up to P.T. Barnum Square (P.T. Barnum not included) Estimated 18,000 sq feet.
• Do not remove entry way to buildings
• Replace any damaged pavers not suitable for reuse estimated 4000 sq. feet
• Widen sidewalk to 5 feet at the following locations. In front of the Gift cottage. In front of 152 Greenwood Ave. Estimate 3,500 sq. feet.
• Alternate: remove and replace paver in P.T. Barnum Square.
• Estimated Schedule of Events
9/28/16 – Site walk meet at Municipal Center, 10:00 a.m.
10/13/16 – Receipt of Proposals at Municipal Center, 11:00 a.m.
10/13/16 – Open Bid Procurement Committee, 11:30 a.m.
Job Requirements
• Compact existing base to Manufacturers spec
• Prep area for new bedding sand to lay pavers
• Re-install pavers to new set grade – following existing pattern
• Area around trees wells to be constructed with a larger mulch bed around tree roots to ensure health of tree – square area.
• All pavers to be set and compacted to manufacturers specs
• Walkway should allow for pitching of water and drainage
Other Requirements
• Make proper arrangements with town and/or police department for any assistance with project
• Contractor will ensure all entrances are clear so businesses can operate as normal.

• Commercial General Liability Insurance
Contractor shall provide a commercial general liability insurance policy that includes products, operations and completed operations. Limits should be at least: Bodily injury and property damage with an occurrence limit of $1,000,000; Personal and advertising injury limit of $1,000,000 per occurrence; General aggregate limit of $2,000,000 (other than products and completed operations); Products and completed operations aggregate limit of $2,000,000.
Such coverage will be primary and shall not contribute in any way to any insurance of self insured retention carried by the Town.
Such insurance shall contain coverage for independent contractors, subcontractors and sub consultants of EC.
Such insurance shall contain contractual liability coverage for liability assumed under an insured contract (including the tort liability of another assumed in a business contract).
• Worker’s Compensation Insurance
The EC shall provide Worker’s Compensation Insurance in the required amount as applies to the State of Connecticut and Employers.
Liability Insurance as follows:
Bodily Injury by Accident – $1,000,000 each accident
Bodily Injury by Disease – $1,000,000 policy limit
Bodily Injury by Disease – $1,000,000 each employee
• Automobile Insurance
The EC shall provide Automobile Insurance in the amounts as follows:
Combined Single Limit Liability, Bodily Injury and Property Damage in the minimum amount of $1,000,000
• Umbrella Insurance
The EC shall provide an Umbrella Insurance in the amounts as follows:
Liability Coverage for each occurrence and in the aggregate in the minimum amount of $2,000,000.
Each Policy of Insurance shall include a waiver of subrogation in favor of the Town of Bethel and shall provide cancellation provisions as required by Connecticut Changes and Nonrenewal Provisions.
The Commercial General Liability, Automobile and Umbrella Liability shall name the Town of Bethel as an additional insured. Each certificate of insurance naming the Town of Bethel as an additional insured
shall be subject to review and approval as to both form and substance by the Town of Bethel’s Insurance Agent.

Sec. 31-53. Construction, alteration or repair of public works projects by state or political subdivision; wage rates; certified payroll. Penalties for violations. (a) Each contract for the construction, remodeling, refinishing, refurbishing, rehabilitation, alteration or repair of any public works project by the state or any of its agents, or by any political subdivision of the state or any of its agents, shall contain the following provision:
“The wages paid on an hourly basis to any person performing the work of any mechanic, laborer or worker on the work herein contacted to be done and the amount of payment or contribution paid or payable on behalf of each such person to any employee welfare fund, as defined in subsection (h) of this section, shall be at a rate equal to the rate customary or prevailing for the same work in the same trade or occupation in the town in which such public works project is being constructed.
Any contractor who is not obligated by agreement to make payment or contribution on behalf of such persons to any such employee welfare fund shall pay to each mechanic, laborer or worker as part of such person’s wages the amount of payment or contribution for such person’s classification on each pay day.”
